Question File

Entromy’s Standard Organizational Health Baseline will serve your organization as a thorough sample of questions, touching on key topics like strategy, business goals and priorities, organizational effectiveness, engagement and collaboration. With the expertise of management consultants, these questions have been thoughtfully curated to yield prescriptive insights and subsequent actionable steps. We recommend preserving this list as much as possible for optimal output and benchmarking opportunities, however, if you require minor edits and/or modifications, please let our team know.

The question file consists of:

  • Question Text: The question text contains the text that survey takers will see when accessing the survey.

  • Key Terms: A shorter summary of the full text for dashboard-level reporting. Key terms are 2 to 5 word phrases that summarize the general themes of the question. For example, the key term for the question, “I am engaged in my role,” might be “Engagement.” The use of key terms helps with final reporting, and allows for higher-level summaries on key issues.

  • Category: Indicating a category helps to enhance the survey reporting results. When reporting is complete, the executive materials will group questions into key categories, which will allow for additional insights and breakouts of performance based on key themes. Available categories currently include “Leadership,” “Communication,” “Collaboration,” and other high-level groupings seen below.

  • Question Types: The question type specifies the formatting for the question. The three most common question types are ‘fixed answer,' 'statement,' and 'list select'. 'Sections' allow for meaningful breaks for the survey respondent.

Participant list

Entromy requires a list of employees containing first name, last name, email for deployment. Please use this template as a guide when compiling your user file. We recommend including additional attributes for survey participants based on the views of data that you’ll want to see in the survey results. For example, if you want to see how results vary by department, manager, supervisor, or location, please make sure that each of these dimensions has its own column in the employee file and they are each fully filled out with accurate data. Please see above for recommended user file dimensions.

Survey Communications

The Survey Communication is integral for participants to be thoroughly involved throughout the survey taking process journey. The launch and reminder communications will come from the Entromy platform, and we also recommend sending internal communications prior to and post survey launch- all of which can be customized. Find all templates here.

IT Safe Sender Confirmation

Corporate spam filters often have strict settings, so it's important that Entromy is whitelisted with your company. The IT Safe Sender Confirmation is essential to help ensure survey emails are delivered to your participants inboxes successfully. Please provide the information below to your IT administrator, and ask for their confirmation of completion.

Confirm that email servers allow for the following email addresses to be considered as safe senders:
