What You’ll Need to Provide
To initiate setup, please gather and send the following components:

Question Type: Includes ‘fixed answer,’ ‘statement,’ or ‘list select’
2. Participant List
Please include:
First name, last name, and email (required)
Any other useful data for filtering results (e.g., department, location, manager)
Make sure all data columns are filled out completely and consistently.
