Setting up a Participant File for a 360 Survey (Pre Launch)

Accessing the Participant Template

o Begin by creating the 360 survey.

o Select "Add Survey" in the configure surveys section.

o Choose the executive 360 template.

Downloading the Participant Template

o Navigate to the participant section of the survey setup.

o Download the participant template from the provided link.

o The template includes instructions and the actual template with rows for each review instance.

Filling out the Participant Template

o Enter the first name, last name, and email address of individuals receiving feedback in columns A-C.

o Include feedback provider's information in columns D-F and relationship designation.

o Fill out the manager review section if applicable, otherwise leave blank.

Notes for setting up your 360 Participant file:

  • There should be 1 row for every instance of a feedback provider giving the recipient feedback.

    • For example: If ‘Sam Smith’ is getting feedback from herself, her manager, and 8 peers, then there would be 10 rows on the participant file with ‘Sam Smith’ listed in all 3 feedback recipient columns.

  • The 360 Participant file should reflect all the feedback recipients who will be associated with the same set of 360 questions.

    • Your initiative should have as many user files as it will question sets.

Importing the Participant File

o Save the filled template and import it back to the platform.

o Map relationships from the imported file.

o The participants' names and feedback providers will be populated on the participants page.

Adding Additional Feedback Providers

o Edit the XLS file to add more feedback providers.

o Save the file and import the updated list.

o Map the correct relationship designation for each individual.

o Save the changes and continue to finalize the setup.

Adding Feedback Providers 1 by 1 from the UI

o Select ‘Add Participants’ – Choose from either the organization if the user already exists in your org in Entromy, or ‘Create New’ to add a new user.

o Search for the individuals and assign their roles (recipient or provider).

o Select the relationship between the feedback provider and recipient.

o Save the changes and continue to finalize the setup.
