1. You can include any employee attributes from the list below. We recommend choosing attributes based on the cuts (views of data) you'll want to see when you get your results. For example, if you want to see how results vary by department, add a field titled Department.

Used to indicate an employee's organization (such as Finance or Operations), or location (such as New Jersey or New York)

Name of employee's manager

Type

Used to further distinguish between types of employees that are meaningful for your organization. For example: Full time / part time; hourly / salary.

"Full time" or "Part time" / "Hourly" or "Salary"

Mobile

Phone number for the survey recipient

Country code + ten-digit number, with no dashes

Kiosk Code

This is a code that can be used if the survey is not emailed but is part of a kiosk or survey-taking station. This can be an employee ID, or email alias, or other unique identifier / code.

Up to 50 characters.

Company Start Date

Start date for the employee. Used to calculate employee tenure (see row below).
